What is the Family Support 360 Program?
A
wide array of services and supports provided to families of children and
individuals with a developmental disability. Supports are designed to
help families get what they need to stay together. Family Support
means different things to different families. No two families are alike
and neither are their needs.

How does Family Support work? |

First
apply to the Family Support statewide program. (Application)

The
statewide program does not provide service coordination and all dollars are
general dollars.
- Once
approved you are put on a waiting list for the local family support program.
- There
are currently 25 local programs; 20 children's programs (up to age 22) and 5
adult programs across the state.

South Dakota Achieve currently has 2 child programs.

What is the role of a Family Support Coordinator?

families who have children with disabilities to remain with their
loved ones, in their own home

Help families get and maintain funding

Assist family/person to identify and
prioritize needs

Connie Becker - Family
Support 360 Coordinator
I have been with
South Dakota Achieve for a total of eleven years. Recently, November of
2008, I transferred to a new career working with children with
disabilities and their families through the Family Support Program. The
mission of Family Support is to help families be successful with caring
for their children with special needs within their own home. The
passion and dedication to this mission is very important to me and I
strive to achieve daily. Prior to becoming a Family Support
Coordinator I also worked as a Service Coordinator, Coordinator of
Support Services in the residential homes at Achieve (both for adults
with disabilities). I also have experience working in Minnesota in the
human services field for eleven years. My husband, Jason, and I have two
daughters. When I’m not working, I enjoy spending time with my family,
friends and having fun.

Stacy Van Horssen-
Family Support 360 Coordinator
The Family Support 360 program is
new to South Dakota Achieve, and I am new to the agency as well. Prior to
my time with SD Achieve, I’ve worked in a helpline call center, provided
project management to a medical company and have been a supervisor at a
crisis domestic violence shelter. I have a Masters of Science in Criminal
Justice from Colorado Technical University and have a Bachelors of Arts in
Criminal Justice and Sociology from the University of Sioux Falls. In my
free time I enjoy spending time with my husband, playing with our energetic
Maltese, visiting with family and friends, traveling and reading a good
book.
