Have you ever wondered how your loved one’s plan for achieving his dream was progressing? Have you ever wished that you could send an immediate message to your loved one’s team? Have you ever wanted to share a new goal or dream that your loved one has? Do you want to know about upcoming meetings, events, and appointments without having to call someone?

Good News….you can!! We have a new on-line system that makes all this and much more possible. This system contains all the information relating to a person in one place. This means that the person, SDA team, and family/guardian have up-to-the minute information available at their finger tips. The system is designed to ensure that all team members can access all information, communicate efficiently, and provide the best services possible to everyone.

Information contained in the new on-line system includes: medical information, emergency contact information, goal progress and accomplishments, all supports in place and all documentation that goes along with each support.

We are excited about this new system and want to share it with all family members/guardians who wish to have access. This is a great way for people to log into our system and see exactly what is being done to ensure the safety, well-being, and personal happiness of a family member or loved one. With this new system, families and guardians are also able to e-mail the entire team or one specific team member once they access the on-line page. This allows you to make comments, ask questions, and make suggestions.

If you are interested in taking part in the on-line process, please contact the Service Coordinator for your family member/loved one. The person served will have to give permission for family and friends to have access to their personal information. Below is a list of the information that the Service Coordinator will need to get this set up for you:

• Email address;
• Password that you wish to use to access the system. This password should be a mix of at least seven characters and numbers; and
• Name of the individual for whom you wish to access information.

Once the system is set, you will receive instructions on how to access the system.
